| Information on Membership Termination |

| Ordinary Membership |
| |
| Two (2) months' written notice is needed in order for the
Club to liaise with the member's payroll department to effect cessation of
salary deduction. |

| Please obtain the membership termination form from the
Front Office of the Clubhouses or send an e-termination notice by clicking
here to print
the Termination Form and fax or mail to us. |

| Co-payment Membership |
| |
| Members will need to inform their organisation's co-ordinator
of their intention to terminate the Club membership, before the beginning of
the effective month of termination. |
